CareStar, Inc. is currently seeking a part time/ casual recovery manager assessment specialist in the Clinical department to conduct program assessments using program and CareStar identified tools and process and create an initial care plan for individuals with severe and persistent mental illness. The Recovery Manager Assessment Specialist is a home based position that works independently including travel to individuals homes

THE POSITION

CareStar is looking for motivated individuals who/to:

• Active license as RN, LSW, or LISW or Bachelor's degree in Social Work, Counseling, Psychology or related field and 
• Minimum of 3 years post degree experience working with individuals with server and persistent mental illness or 
• Minimum of 1 year post degree experience working with individuals with diagnosed chronic conditions
• Adheres to the CareStar Rule in performance of job responsibilities.
• Understands and complies with CareStar Policies and Procedures.
• Maintains confidentiality as related to patient information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense.

• Follows the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc.

This position is open in Hamilton, Butler and Montgomery County and is responsible for covering the surrounding counties.
