CareStar an established leader in the home and community-based healthcare industry in Ohio, Indiana and Georgia is looking for a community-based CASE MANAGER -  SPECIALIZED RECOVERY PROGRAM for  the Specialized Recovery Services Program (SRSP) in Ohio.  The Case Manager will determine eligibility for the program and provide ongoing care planning and case management for individuals with serious mental illness and/or diagnosed chronic conditions. This is a home-based position that requires travel to individual's homes.
 
Qualified candidates will have the following knowledge and experience:

 * Active license as RN, LSW, or LISW.  OR  Bachelor's degree in Social Work, Counseling, Psychology or related field and
 * Minimum 3 years experience (post degree) working with persons with severe and persistent mental illness or
 * Minimum of 1 year post degree experience working with individuals with diagnosed chronic conditions.
  
It is preferred that Case Manager - Specialized Recovery Program reside within the counties of coverage.
