Assessment Specialist (Specialized Recovery Services Program) - Cuyahoga and Surrounding Counties

CareStar, Inc. is currently seeking an Assessment Specialist in the Operations Department to conduct assessments the Specialized Recovery Services (SRS) Program using program and CareStar identified tools and processes. The Assessment Specialist will provide recommendations for eligibility per program and CareStar guidelines.


THE POSITION

CareStar is looking for motivated individuals who/to:

  • Maintains and builds knowledge of the State and program requirements.
  • Maintains and builds on knowledge of Medicare, third-party payers, community resources, and the Americans with Disabilities Act.
  • Completes in-home visits to assess for eligibility for specified program and conducts safety assessments, in compliance with CareStar and department guidelines.
  • Documents all applicant and case-related contacts and interventions in applicants record per agency and CareStar policy, including standards of content and timeliness.
  • Provides assistance with community resource referrals as appropriate.
  • Possesses a Bachelor's Degree in social work, counseling, psychology, or a related field; or Registered Nurse (RN), or Licensed Social Worker (LSW).
  • Possesses a minimum of one-year post degree experience working with individuals with diagnosed chronic conditions and/or minimum of three years’ post degree experience working with individuals with severe and persistent mental illness.
  • Knowledge of issues affecting people with severe and persistent mental illness and community-based interventions/resources for those individuals.
  • Effective interpersonal (oral and written) skills to interact professionally and ethically with all age groups from a diverse population, including differing intellectual, cultural, ethnic, psychosocial and socio-economic backgrounds.
  • Adheres to the CareStar Rule in performance of job responsibilities.
  • Understands and complies with CareStar Policies and Procedures.
  • Maintains confidentiality as related to patient information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense.
  • Follows the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc.

This position is open in Cuyahoga County and is responsible for covering the surrounding counties.
