The Community Health Worker will serve as an intermediary between Health and Social Services to facilitate access to health services to remove social, cultural and environmental barriers and thereby improve identified health outcomes, including, but not limited to, preventing hospital readmissions.

Job Roles and Responsibilities

  • Adheres to the CareStar Rule in performance of job responsibilities.
  • Understands and complies with CareStar Policies and Procedures.
  • Maintains confidentiality as related to patient information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense.
  • Follows the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc.
  • Acts as point of contact with individual to engage with individual in his/her home and/or community.
  • Identifies barriers to compliance with individual’s healthcare regime, appointments and screenings.
  • Works with individual to create solutions to identified barriers and empowers individuals to manage physician, and/or other key medical professionals, contacts, appointments and/or screenings.
  • Communicates and collaborates with individual’s Health Care Team to assist to improve quality and cultural competence of service delivery.
  • Sources and refers individuals to appropriate community resources.
  • Completes all required contacts, documentation, assessments per program requirements.
  • Serves on work groups, community groups, committees, or other task forces and attend meetings as requested.
  • Performs other projects and duties as assigned.

 

Job Qualifications

  • Bachelor’s Degree; or an Associate’s Degree and two years’ relevant experience; if no degree, five years’ relevant experience.
  • At least three years’ experience working specifically with Children and Youth with Special Health Care Needs (CYSHCN) in a community setting.
  • Reside within the assigned coverage area.
  • Knowledge of resources for CYSHCN residing in Pennsylvania.
  • Demonstrates the ability to access resources for CYSHCN residing in Pennsylvania.
  • Over the age of 18.
  • Possess a valid Pennsylvania driver’s license.

 

THE LOCATION

This position is open in Philadelphia County and is responsible for covering the surrounding counties.
