Community Health Worker Flex- Summit County

CareStar, Inc. is currently seeking a Community Health Worker in the Clinical department to assist kids in the Barberton School Systems using program and CareStar identified tools and process. As well, the Community Health Worker will assist children and their families in gaining access to needed services, as well as medical, social, educational, and other resources, regardless of funding source. The Assessment Specialist is a home based position that works independently to manage a caseload as needed.

​​​​​​​Flex staff as employed on an "as needed" basis. When his or her services are required by CareStar, Inc., the employee may work up to a maximum of 29 hours per week. Employees in this category are not eligible for benefits unless otherwise set forth in the Employee Benefit Eligibility Policy, but are eligible for mileage reimbursement pursuant to CareStar policy.

THE POSITION

CareStar is looking for motivated individuals who/to:

• Have experience working with youth, and their families, who meet a functional need for behavioral healthcare.
• Able to work After School Hours as well as one weekend day.
• 12 months experience in home and community based services within the past 5 years.
• Knowledge of Medicare, Medicaid, home health care and Ohio Home Care waiver programs desirable.
• Adheres to the CareStar Rule in performance of job responsibilities.
• Understands and complies with CareStar Policies and Procedures.
• Maintains confidentiality as related to patient information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense.Follows the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc.

This position is open in Summit County, primarily in Berberton City School District.
