Description

CareStar an established leader in the home and community-based healthcare industry is currently seeking an Intensive Support Coordinator  to identify, coordinate and oversee the delivery of services to enhance the health, safety and general wellbeing of participants to assist the participants goals toward maximum independence.

Job Duties Include but are not limited too:
Ensures that participant’s personal profile is complete, accurate and up-to-date per program and CareStar requirements.
• Maintains knowledge, understanding and compliance with current program rules, requirements and policies.
• Maintains knowledge base of Medicare, Medicaid, insurance and program administered waivers.
• Maintains knowledge of the Americans with Disabilities Act.
• Functions as the team leader in the development and implementation of the participant's individual service plan, including the authorization of the amount, scope and duration of services.
• Conducts health and safety, social and environmental assessments per established guidelines.
• Provides information and assistance that helps the participant (and/or representative) in making informed decisions regarding available options.
• Links participants to needed waiver services and/or providers to address identified needs.
• Takes active measures to develop information regarding non-waiver funded services and/or supports available in the community.
• Provides information and assistance to link participants with medical, social and/or educations providers and/or other programs and services that can provide needed services to address identified needs and achieve identified goals.
• Evaluates the satisfaction of participants (and their families) with the individual service plan and its implementation; reviews the service plan delivery to ensure there are no unmet goals.
• Monitors the health and welfare goals of the participant through direct observation, reviews documentation and follow-up to ensure that service plans have the intended effect and that approaches to address challenging behaviors, medical and health needs, and skill acquisitions are coordinated in their approach and anticipated outcome.
• Documents all consumer and case-related contracts and interventions in the consumer’s communication notes per agency policy, including the standards of content and timeliness.
• Participates in quality management reviews, audits, Program Eligibility and Assessment Tool (PEAT) and program tools to review activities as requested.
• Attends orientation, trainings, in-services, consumer care conferences, and staff meetings as determined by regulatory standards and Management.
• Conducts or facilitates the completion of the Supports Intensity Scale (SIS) assessment as required.
• Reviews the Health Risk Screening Tool (HRST), including all service considerations and training recommendations, to incorporate assessed needs and individual specific health and safety considerations in the individual service plan.
• Reviews all healthcare plans, positive behavior support plans (PBSPs), crisis plans and safety plans, including a review tracking documentation.
• Completes the annual Level of Care Authorization Form (DMA)-7 level and other program required forms per program and CareStar requirements to facilitate level of care re-evaluation.

QUALIFICATIONS AND EDUCATION REQUIREMENTS
• Bachelor's Degree in Human Service Field such as Social Work, Psychology, Sociology, Counseling, Gerontology or Nursing (or closely related field).
• Two years of direct service experience serving persons with Developmental Disabilities (DD)/Intellectual Developmental Disabilities (IDD) or related field. 
• Experience in healthcare, habilitative/rehabilitative, residential, behavioral health setting (Intermediate Care Facility (ICF)/ Intellectual Developmental Disabilities (IDD), psychiatric inpatient hospital/facility, specialty clinic, special education department or similar setting.
• Experience in completing assessments, service plan development and monitoring; Experience providing care management/coordination preferred.
• Excellent verbal and written communication skills.
• Willingness and ability to work flexible schedule to meet needs of families. 
• Ability to work independently; effective organizational skills, including prioritizing projects and attention to detail.
